By creating a personalized debit card, you give Patriot Bank permission to use the photograph on the card. By submission of your application you agree to hold harmless and indemnify Patriot Bank from any loss, damages, liability, or costs should any be incurred directly or indirectly by the use of the photograph. You agree to destroy your old card once you receive your new card and hold Patriot Bank harmless for any loss as a result of not doing so.

It's your responsiblity to print a copy of the terms and conditions for your records.
